My husband's high-index progressive glasses were $100 out of pocket with another $100 paid by insurance. Anti scratch and anti reflective coatings are included in the standard lens prices. 

My eyeglasses from Costco w/high index lenses were btwn $150-$200 - I don't recall the exact cost - they don't have a huge selection of frames but I wasn't looking for anything special, and I have to choose certain styles just due to my correction.
